The aim of this book is to reduce the risks of medical treatment and enhance the safety of patients in all areas of healthcare. The first section discusses human error, the incidence of harm to patients, and the development or risk management . Chapters in the second section discuss the reduction of risk in clinical practice in key medical specialties. The third section discusses features of the healthcare systems that are essential to safe practice, such as communication of risk to patients, the design of equipment, supervision and training, and effective teamwork. The fourth section describes how to put risk management into practice, including the effective and sensitive handling of complaints and claims, the care of injured patients and the staff involved, and the reporting, investigation and analysis of serious incidents.

Malignant : How Bad Policy and Bad Evidence Harm People with Cancer

How hype, money, and bias can mislead the public into thinking that many worthless or unproven treatments are effective.Each week, people read about new and exciting cancer drugs . Some of these drugs are truly transformative, offering major improvements in how long patients live or how they feel—but what is often missing from the popular narrative is that, far too often, these new drugs have marginal or minimal benefits. Some are even harmful. In Malignant, hematologist-oncologist Dr. Vinayak K. Prasad writes about the many sobering examples of how patients are too often failed by cancer policy and by how oncology is practiced. Throughout this work, Prasad illuminates deceptive practices which• promote novel cancer therapies long before credible data are available to support such treatment; and• exaggerate the potential benefits of new therapies, many of which cost thousands and in some cases hundreds of thousands of dollars.Prasad then critiques the financial conflicts of interest that pervade the oncology field, the pharmaceutical industry, and the US Food and Drug administration . Over 98% of sprayed insecticides and 95% of herbicides reach a destination other than their target species, including non-target species, air, water and soil. The extensive reliance on insecticide use reduces biodiversity, contributes to pollinator decline, destroys habitat, and threatens endangered species. This book offers a more effective application of the Integrated Pest Management (IPM) approach, on an area-wide (AW) or population-wide (AW-IPM) basis, which aims at the management of the total population of a pest, involving a coordinated effort over often larger areas. For major livestock pests, vectors of human diseases and pests of high-value crops with low pest tolerance, there are compelling economic reasons for participating in AW-IPM. RESEARCHER ACCESS TO SOCIAL MEDIA DATA: LESSONS FROM CLINICAL TRIAL DATA SHARING.

For years, social media companies have sparred with lawmakers over how much independent access to platform data they should provide researchers. Sharing data with researchers allows the public to better understand the risks and harms associated with social media, including areas such as misinformation, child safety, and political polarization. Yet researcher access is controversial. Privacy advocates and companies raise the potential privacy threats of researchers using such data irresponsibly. In addition, social media companies raise concerns over trade secrecy: the data these companies hold and the algorithms powered by that data are secretive sources of competitive advantage. This Article shows that one way to navigate this difficult strait is by drawing on lessons from the successful governance program that has emerged to regulate the sharing of clinical trial data. Like social media data, clinical trial data implicates both individual privacy and trade secrecy concerns. Nonetheless, clinical trial data's governance regime was gradually legislated, regulated, and brokered into existence, managing the interests of industry, academia, and other stakeholders. The result is a functionally successful (albeit imperfect) clinical trial data-sharing ecosystem. Part II sketches the status quo of researchers' access to social media data and provides a novel taxonomy of the problems that arise under this regime. Part III reviews the legal structures governing sharing of clinical trial data and traces the history of scandals, investigations, industry protest, and legislative response that gave rise to the mix of mandated sharing and experimental programs we have today. Part IV applies lessons from clinical trial data sharing to social media data and charts a strategic course forward. Three primary lessons emerge: first, the benefits of research on otherwise secret data are cascading and unpredictable; second, law without institutions to implement the law is insufficient; and, third, data access regimes must be tailored to the different sorts of data they make available. The South African Response to COVID-19 : The Early Years

This book analyses the first two years of South Africa's response to the COVID-19 epidemic, from its emergence in early 2020. Drawing on the perspectives of a range of public health experts, economists and other social scientists, and development practitioners, this book argues that understanding this early response will be essential to moderate and improve future policy thinking around health governance and epidemic readiness. This book provides a systemic analysis of not only the epidemiological progression of COVID-19 in South Africa, but also the socio-political factors that will be key in determining the future of the country as a whole, including health system challenges, socio-economic disparities and inequalities, and variable (often contradictory and tardy) policy responses. Overall, this book exposes Manichean thinking and the spurious policy dichotomies that pitch public health against human rights, economic recovery against viral vector control, and science against ideology, with lessons not just for South Africa, but also for elsewhere on the African continent, and beyond. Chapter 1 provides background information on e-cigarettes. Tobacco use among youth and young adults in any form, including e-cigarettes, is not safe. In recent years, e-cigarette use by youth and young adults has increased at an alarming rate. E-cigarettes are now the most commonly used tobacco product among youth in the United States . Chapter 2 highlights the rapidly changing patterns of e-cigarette use among youth and young adults, assesses what we know about the health effects of using these products , and describes strategies that tobacco companies use to recruit our nation's youth and young adults to try and continue using e-cigarettes. It also outlines interventions that can be adopted to minimize the harm these products cause to our nation's youth. Over the last decade, use of electronic cigarettes, known as e-cigarettes, in the United States has grown rapidly as use of traditional cigarettes declined among both adolescents and adults. Most e-cigarettes sold in the United States were thought to be imported, but e-cigarette import volume and tariff revenue were unknown because the Harmonized Tariff Schedule of the United States (HTS) did not contain separate statistical reporting numbers. New statistical reporting numbers went into effect on January 1, 2016. Chapter 3 reports on e-cigarettes imported into the United States in 2016. While chapter 4 reports on e-cigarettes imported into the United States from 2016 through 2018. E-cigarettes are battery-powered devices which simulate tobacco smoking by producing a heated vapor that resembles smoke. A number of explosions and fires have been attributed to e-cigarettes, causing some concern within the fire service community as discussed in chapter 5. Electronic cigarettes, known as “personal vaporizers” (PV) have experienced a significant increase in popularity for those seeking an alternative to smoking traditional tobacco products . The use of the e-cigarettes as an illicit drug delivery device is touted on websites, forums, blogs, and videos describing how best to use them for specific illicit drugs such as tetrahydrocannabinol, methamphetamine, fentanyl, and heroin as reported in chapter 6. Chapters 7 and 8 report on the 2019 outbreak of lung injury associated with e-cigarette use .

The sterile insect technique (SIT) is an environment-friendly method of pest control that integrates well into area-wide integrated pest management (AW-IPM) programmes. This book takes a generic, thematic, comprehensive, and global approach in describing the principles and practice of the SIT. The strengths and weaknesses, and successes and failures, of the SIT are evaluated openly and fairly from a scientific perspective. The SIT is applicable to some major pests of plant-, animal-, and human-health importance, and criteria are provided to guide in the selection of pests appropriate for the SIT. In the second edition, all aspects of the SIT have been updated and the content considerably expanded. A great variety of subjects is covered, from the history of the SIT to improved prospects for its future application. The major chapters discuss the principles and technical components of applying sterile insects. The four main strategic options in using the SIT — suppression, containment, prevention, and eradication — with examples of each option are described in detail. Other chapters deal with supportive technologies, economic, environmental, and management considerations, and the socio-economic impact of AW-IPM programmes that integrate the SIT. In addition, this second edition includes six new chapters covering the latest developments in the technology: managing pathogens in insect mass-rearing, using symbionts and modern molecular technologies in support of the SIT, applying post-factory nutritional, hormonal, and semiochemical treatments, applying the SIT to eradicate outbreaks of invasive pests, and using the SIT against mosquito vectors of disease. Risk assessment has become a dominant public policy tool for making choices, based on limited resources, to protect public health and the environment. It has been instrumental to the mission of the U.S . Environmental Protection Agency (EPA) as well as other federal agencies in evaluating public health concerns, informing regulatory and technological decisions, prioritizing research needs and funding, and in developing approaches for cost-benefit analysis. However, risk assessment is at a crossroads. Despite advances in the field, risk assessment faces a number of significant challenges including lengthy delays in making complex decisions; lack of data leading to significant uncertainty in risk assessments; and many chemicals in the marketplace that have not been evaluated and emerging agents requiring assessment. Science and Decisions makes practical scientific and technical recommendations to address these challenges. This book is a complement to the widely used 1983 National Academies book, Risk Assessment in the Federal Government (also known as the Red Book). The earlier book established a framework for the concepts and conduct of risk assessment that has been adopted by numerous expert committees, regulatory agencies, and public health institutions. This collection advocates languages-based, translational research to be part of the partnerships and collaborations required to make sense of, and respond to, COVID-19 as one of the major global challenges of our time. Bringing together scholars and practitioners from a wide range of disciplines, this volume is bound by a common thread stressing the importance of linguistic sensitivity, (inter)cultural knowledge and translational mediation in the frontline response to COVID-19. Featuring contributors from around the world and reflecting on the language used to frame COVID-19 in diverse cultural contexts of the Global North and Global South, the book proposes that paying attention to the transmission of ideas, ideologies, narratives and history through processes of translation results in a broadening of social, cultural and medical understandings of COVID-19. COVID-19 has brought unprecedented challenges in the care of older adults. During the first surge of the pandemic, governments all over the world struggled with high disease severity and increased mortality among older adults. This work documents the impact of the pandemic by collating information from different countries and by synthesizing inputs from several knowledge domains—Sociology, Gerontology, Geriatrics, Medicine and Public Health. The impact on older adults is examined primarily with respect to three main issues—pervasive ageism, spread of infections in care homes worldwide, and the unintended harm of public health measures on geriatric population in different care settings. The complex tensions between epidemic control and the need to respond to social and economic imperatives are investigated with respect to disadvantaged and vulnerable older adults. The book also critically examines international ageing policies with the intention of identifying gaps in pandemic response in particular, and approaches to older adult care in general. In the light of the evidence presented, lessons are drawn which might improve aged care and strengthen emergency preparedness. Finally, considering the evolving nature of the pandemic, new international responses to older adult care and pandemic management are presented as an epilogue. The COVID-19 pandemic has increased the focus on health informatics and healthcare technology for policy makers and healthcare professionals worldwide. This book contains the 110 papers (from 160 submissions) accepted for the 18th annual International Conference on Informatics, Management , and Technology in Healthcare (ICIMTH 2020), held virtually in Athens, Greece, from 3 – 5 July 2020. The conference attracts scientists working in the field of Biomedical and Health Informatics from all continents, and this year it was held as a Virtual Conference, by means of teleconferencing, due to the COVID-19 pandemic and the consequent lockdown in many countries around the world. The call for papers for the conference started in December 2019, when signs of the new virus infection were not yet evident, so early submissions were on the usual topics as announced. But papers submitted after mid-March were mostly focused on the first results of the pandemic analysis with respect to informatics in different countries and with different perspectives of the spread of the virus and its influence on public health across the world. This book therefore includes papers on the topic of the COVID-19 pandemic in relation to informatics reporting from hospitals and institutions from around the world, including South Korea, Europe, and the USA . The book encompasses the field of biomedical and health informatics in a very broad framework , and the timely inclusion of papers on the current pandemic will make it of particular interest to all those involved in the provision of healthcare everywhere.

This book provides an overview of the current epidemiology of the HIV epidemic among young people in Eastern and Southern Africa (ESA) and examines the efforts to confront and reduce the high level of new HIV infections amongst young people. Taking a multi-dimensional approach to prevention, the contributors discuss the many challenges facing these efforts, in view of the slow progress in curbing the incidence of HIV amongst young people, focusing particularly on the structural and social drivers of HIV. Through an examination of these issues, chapters in this book provide valuable insights on how to mitigate HIV risk among young people and what can be regarded as the catalysts to mounting credible policy and programmatic responses required to achieve epidemic control in the region. The contributors draw on examples from a range of primary and secondary data sources to illustrate promising practices and challenges in HIV prevention, demonstrating links between conceptual approaches to prevention and lessons learnt from implementation projects in the region. Bringing together social scientists and public health experts who are actively engaged in finding effective solutions, the book discusses ‘which interventions works', ‘why they work', and the limitations and gaps in our knowledge to curb the pandemic amongst young people. As such it is an important read for researchers focusing on HIV/AIDS and public health. Through a series of studies, the overarching aim of this book is to investigate if and how the digitalization/digital transformation process affects various welfare services provided by the public sector, and the ensuing implications thereof. Ultimately, this book seeks to understand if it is conceivable for digital advancement to result in the creation of private/non-governmental alternatives to welfare services, possibly in a manner that transcends national boundaries. This study also investigates the possible ramifications of technological development for the public sector and the Western welfare society at large. This book takes its point of departure from the 2016 Organization for Economic Co-operation and Development (OECD) report that targets specific public service areas in which government needs to adopt new strategies not to fall behind. Specifically, this report emphasizes the focus on digitalization of health care/social care, education, and protection services, including the use of assistive technologies referred to as'digital welfare.'Hence, this book explores the factors potentially leading to whether state actors could be overrun by other non-governmental actors, disrupting the current status quo of welfare services. The book seeks to provide an innovative, enriching, and controversial take on society at large and how various aspects of the public sector can be, and are, affected by the ongoing digitalization process in a way that is not covered by extant literature on the market. This book takes its point of departure in Sweden given the fact that Sweden is one of the most digitalized countries in Europe, according to the Digital Economy and Society Index (DESI), making it a pertinent research case. However, as digitalization transcends national borders, large parts of the subject matter take on an international angle. This includes cases from several other countries around Europe as well as the United States .
